Health Insurance & Immunization Information

Below are details on the health insurance program offered through Hamline: 

This is a basic 90%/10% Network Provider accident and illness policy.  The student is responsible for one $50.00 deductible per year and pays 10% of the covered charges; the insurance company pays 90% (after the deductible is paid) of the covered charges up to $100,000 per accident or illness.

This is an accident and illness policy, so it does not cover most preventative care.  It will cover an annual pap test.

Most mental health services are covered.

There is a $10.00 co-pay for generic medication, $25.00 for brand name, and $35.00 for single source drugs per prescription.  The insurance will pay for prescriptions up to $1000.00 per illness. Birth control is payable up to $300.00 per policy year. Discount cards for prescriptions, dental and eye care are available. 

In some cases the student will be asked to pay at the time of service.

As with all insurance, some services are not covered. These are listed in the brochure under exclusions.

                             IMMUNIZATIONS

Minnesota Law requires that all students born after 1956 and enrolled in a public or private post-secondary school in Minnesota be immunized against diphtheria, tetanus, measles, mumps and rubella.  Immunization forms can be accessed online by clicking on the link below.  Forms may be mailed to or dropped off at our office.  Additionally, students can enter in their immunization information on Piperline under Services for Admitted Students.  Until the immunization form is turned into Counseling & Health Services or immunization information is entered into Piperline, you will not be able to register.
